I totally agree with Attonine. The equipment should be designed to handle power cycle stress. A 3% expected failure is rather a high figure in my book. It is extremely difficult to diagnose your problem without having hands on with the equipment. Looking at the table you have produced I would guess that the issue is with the input of the amplifier.
